What support is available to help me keep independent?
If you have a disability and/or experience difficulty completing daily tasks, you have a couple of options:
Community Equipment Service
The service provides a range of equipment to help you live independently, whether it’s equipment for your home, or things to help you while out and about. This funded by your local Council and NHS, where Medequip provide the equipment on loan and includes delivery and fitting, servicing, collection and recycling.
You may be eligible for specific equipment following an assessment, where health or social care prescribers such as an Occupational Therapist can work with you to find ways of managing tasks, offer advice and potential solutions.
To access equipment and request an assessment, you can ask your GP for a referral or if you are to be discharged from hospital, an assessment can be arranged. To enquire or request an assessment directly, you can contact your Council's Social Care Team:
Norfolk County Council resident: call 0344 800 8020 or complete the self-referral form online.
Suffolk County Council resident: call The Care Coordination Centre (CCC) on 0300 123 2425 or complete the self-referral form online.
Returning Equipment
If you have already benefited from equipment loaned to you that you no longer need, you can request a free collection so Medequip can reuse and help others in need. Contact us on 01603 511124 or email norfolk@medequip-uk.com.
Alternatively, you can return equipment directly to our service centre.
